If you would like to receive approvals, please complete the form below. and click on Submit to e-mail it to us. If you prefer, make a copy of this form and mail or fax it to us. As soon as we have a selection on your topic(s), we will ship it to you by certified or registered mail. Your only obligations are to pay for the material you wish to keep within 15 days of receipt and / or to return the material you do not wish to keep by certified or registered airmail. You may keep the entire shipment, only some items, or nothing. You may request to stop approval shipments at any time. No risk. No obligations. DO NOT HESITATE.
